For PTA events and initiatives, please make use of these forms to aid in communications with our Poly Drive families.
Pre event planning form
Provides basic information about the event that will be used in our communications channels (school newsletter, Facebook, this website)
Submit the form at least three weeks in advance of the event/initiative launch
Communications request form
Make use of the Communications Committee for publicity assistance with your event/initiative launch. Submit the form at least three weeks in advance of the event/initiative launch.
Poly Drivers post event info form
Spread the word about your successful event or initiative! This sheet is the follow-up piece, used for the Poly Drivers section of this website and other communications efforts.
Submit within a month of the event/initiative conclusion.
